Hematological and immune disorders in colorectal cancer patients with liver metastases after radiofrequency ablation

چکیده
Background. Colorectal cancer (CRC) is one of the most common malignant neoplasms in world. It ranks third structure oncological morbidity and second mortality. The liver site metastatic spread CR 14 to 18% patients have metastases at diagnosis. Radiofrequency ablation currently promising method treating lesions.
Рurpose – study quantitative changes hematoimmunological indicators their influence on antitumor reactivity with colorectal after RFA metastases.
Materials Methods. Clinical laboratory examination was carried out 12 liver, majority were over 60 years old. Adenocarcinoma histologically determined all patients, them had a moderate degree malignancy (G2). three stages: I day before radiofrequency (RFA), II 3 days RFA, III Results. Immune hematological development inflammatory response established. On 3rd (II stage) significant increase total number leukocytes, ratio neutrophils lymphocytes, eosinophils, violation balance CD4+ CD8+ decrease NK- NKT-cells, an percentage CD3+ HLA-Dr -, CD4 +PD1+, CD8+PD1+ lymphocytes. 14th (stage III), parameters close those treatment, except for reduced NK NKT cells level platelets.
Conclusions. A related reaction identified: neutrophils/lymphocytes from 1.72 4.12, relative absolute eosinophils. But day, subsided, normalization studied observed. subpopulation composition lymphocytes established RFA. Those that taken place even intervention (low cells) additional ones appeared (increased CD4+/CD8+ ratio, increased HLA-Dr+, PD1+ (CD279+), cells). picture approached initial one, exception cells. We believe timely correction immunosuppression early can shorten period vulnerability recurrence CR, long term potentiate positive effect reactivity.

Radiofrequency ablation for colorectal liver metastases.
The management of hepatic metastases from colorectal cancer (HMCRC) is multimodal including chemotherapy, surgical resection, radiation therapy, and focused destruction technologies. Radiofrequency ablation (RFA) is the most commonly used focused destruction technology. It represents a therapeutic option that may be potentially curative in cases where surgical excision is contra-indicated. It a...

BACKGROUND To evaluate a strategy combining high-dose 5FU-irinotecan-leucovorin (HD-FOLFIRI) chemotherapy, radiofrequency ablation and surgery in patients with unresectable liver metastases from colorectal cancer. PATIENTS AND METHODS Patients, all presenting UDP glucuronosyl transferase-1A1 (UGT1A1) 6/6 or 6/7 genotype, received HD-FOLFIRI (with high-dose irinotecan: 260 mg/m(2)), one cycle ...
متن کاملLocal tumor progression patterns after radiofrequency ablation of colorectal cancer liver metastases.
PURPOSE We aimed to evaluate patterns of local tumor progression (LTP) after radiofrequency ablation (RF ablation) of colorectal cancer liver metastases (CRCLM) and to highlight the percentage of LTP not attributable to lesion size or RF ablation procedure-related factors (heat sink or insufficient ablation margin). METHODS CRCLM treated by RF ablation at a single tertiary care center from 20...
متن کاملOutcomes after resection and/or radiofrequency ablation for recurrence after treatment of colorectal liver metastases.
BACKGROUND Repeat liver resection for colorectal liver metastases (CRLMs) is possible in a limited number of patients, with radiofrequency ablation (RFA) as an alternative for unresectable CRLMs. The aim of this study was to analyse survival rates with these interventions. METHODS This was a database analysis of patients who underwent first and repeat interventions for synchronous and metachr...
متن کامل[Radiofrequency ablation of liver metastases].
The liver is the second only to lymph nodes as the most common site of metastatic disease irrespective of the primary tumor. Up to 50% of all patients with malignant diseases will develop liver metastases with a significant morbidity and mortality. Although the surgical resection leads to an improvement of the survival time, only approximately 20% of the patients are eligible for surgical inter...
